The National Nurse-Led Care Consortium is providing the Safe and Healthy Homes Program, funded by the Pennsylvania Department of Health. Services are available in Philadelphia and Delaware counties. This program works in partnership with several agencies in the region and strives to reduce illness and injury caused by home health and safety hazards such as fire and fall hazards, asthma triggers, and lead.
The main activity of this program is to provide free primary prevention home visiting services to reduce risks associated with unhealthy homes.

Households with an asthmatic resident will receive supplies to aid in asthma trigger reduction, such as mattress and pillow covers. In addition, safety supplies such as first aid kits, drawer latches, and furniture anchors may be provided to households, as needed. We do not provide pest extermination services.
NNCC and its partners expect to serve approximately 375 households with primary prevention activities annually.
