Health Outcomes

Health centers require access to information about promising health care practices in order to ensure that care is delivered in an efficient and sustainable manner, that patient outcomes are monitored and efforts put in place to improve them, and that center operations at all levels support performance. NNCC provides health centers with the tools and training to develop and deploy optimized care teams and adopt innovative clinical models to improve patient health.

  • Transforming Practice: By 2019, payments to health care providers will be closely tied to quality metrics. To help prepare health centers for this transition in payment, NNCC is partnering with its in-house Transforming Clinical Practice Initiative (TCPi) to develop and deliver trainings and resources for practice transformation including the formation and optimization of interdisciplinary care teams.
  • HCV Testing, Linkage to Care, and Treatment: NNCC uses its firsthand experience with implementing a successful Hepatitis C testing, linkage to care, and treatment program to support health centers interested in improving their capacity to implement successful HCV programming. As HCV infection rates continue to rise, health centers are poised to support not only testing and linkage services but also treatment. NNCC works with key stakeholders to understand the strengths and challenges to HCV programming at health centers and works with interested health centers and PCAs to identify opportunities for improved programming.
